What Is the Hospital Discharge Medicines Review (DMR) Service?

The Hospital Discharge Medicines Review (DMR) is an Advanced NHS Pharmacy Service unique to Wales. It supports your recovery after a hospital stay by helping to clarify and manage any changes to your medication.

Your review is completely confidential and can take place in our private consultation room, over the phone, or online. You’re welcome to bring someone with you, or if you have a carer, they can attend or speak on your behalf.

How Much Does a Discharge Medicines Review Cost?

If you qualify for this NHS service, there is no cost to you. You can register for this service at any time by speaking to a member of our pharmacy team or by emailing us.

Once registered, any electronic hospital discharge information will automatically be received by our pharmacy team to ensure prompt follow-up and support.

How Long Does the Review Take?

A typical NHS Discharge Medicines Review takes around 10–15 minutes, depending on how many medicines you take and whether there have been any recent changes during your hospital stay.

What Will the Pharmacist Ask Me?

  • Which medicines you are currently taking – we’ll have a record of what we’ve dispensed, but not hospital-issued medicines unless already registered for DMR.
  • Whether you have enough supply of each medicine so you don’t run out.
  • If you experience any difficulties using or taking your medicines.
  • Whether your medicines are being taken regularly and at the correct times.

What Happens After the Review?

  • We’ll follow up with a call or appointment 28–56 days after your hospital discharge.
  • If needed, we’ll refer any issues to your GP for further review or adjustment.
